Overview

Under direction, to perform work providing psychosocial assessment, intervention and monitoring for patients and families that are experiencing a cancer or neurological event. Recommends appropriate programs, resources and interventions to facilitate the patient process. Participates as a team member on various committees as assigned.

Responsibilities

PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions)


  1. Provides a timely, comprehensive and accurate assessment of the psychosocial health and needs of the patient and/or family and recommends appropriate interventions.

  2. Re-evaluates patient at specified intervals to assess status and any follow up needs.

  3. Documents all clinical assessments and interventions in the electronic medical record and communicates the outcomes to the appropriate team members.

  4. Prepares a summary of all patient encounters, reporting summary to the appropriate committees, including Cancer Committee, Breast Leadership Team, and Neuroscience meetings.

  5. Provides patient and family professional counseling within their scope of practice.

  6. Facilitates and leads assigned patient support groups.

  7. Provides education and training of staff regarding the assessment and management of psychosocial patient needs.

  8. Maintains compliance with program accreditation standards set forth by American College of Surgeons Commission on Cancer, National Accreditation Program for Breast Centers, and National Multiple Sclerosis Society for Designation as MS Center.

  • Must demonstrate excellent interpersonal and problem-solving skills.
  • Must be able to work independently and as a member of a multidisciplinary team.
  • Must be flexible in responding to unanticipated demands and priorities.
  • Must present in a compassionate and professional manner.

EDUCATION
  • A Master's Degree from a recognized college or university in Social Work or a closely related field is required.
  • Masters Social Work Required
  • Licensed Clinical Social Work
