Overview

Under direction, to perform work establishing relationships with clients in the hospital and in the community to facilitate a partnership designed to improve self-care abilities and achieve optimal health for the client utilizing techniques such as hospital visits, phone calls, home visits and group classes.

Responsibilities

PRINCIPAL DUTIES AND RESPONSIBILITIES(*Essential Functions)


  1. Works collaboratively with the inpatient and outpatient health care team to coordinate seamless care for clients customized to their individual situations and needs.

  2. Establishes collaborative partnerships with clients and families to assist them in examining patterns of their health care needs and decisions, lifestyle choices and utilization of resources that affect their health.

  3. Educates and coaches clients and families to help them develop self-care skills and independence appropriate to their age and developmental level which may include their primary caretakers.

  4. Collaborates with client and family health care providers and agencies using a multidisciplinary and holistic approach to help the client and family set realistic and achievable goals to achieve the desired outcomes.

  5. Utilizes professional and community knowledge and influence to obtain resources in the most cost effective manner to best meet the client's developmental, physical, psychosocial, environmental, spiritual, cultural and financial needs.

  6. Participates in the development of disease management strategies, measurement of patient outcomes related to hospital utilization, community outreach presentations, education, and groups related to disease management issues.

Qualifications

JOB SPECIFICATIONS(Minimum Requirements)

    KNOWLEDGE, SKILLS, AND ABILITIES
  • Knowledge of national treatment protocols, guidelines and standards of care for the management of patients with the disease of the assigned specialty area.
  • Demonstrated clinical competency in patient care delivery, teaching, and management of patients and families with the disease of the assigned specialty area.
  • Excellent oral and written communication skills.
  • Ability to work independently, with minimal direct supervision, and as a member of multidisciplinary team.
  • Ability to provide compassionate, kind, and caring approach in patient interactions.

EDUCATION
  • Graduation from a recognized college or university with a Bachelor's degree in Nursing or a closely related field is required.
  • Bachelors Nursing Required
  • Four (4) years of progressively responsible clinical registered nurse work experience in acute care setting with responsibilities in teaching and managing patients with a disease state related to the specialty area involved is required.
  • 4 Healthcare/Medical - Acute Care Required
  • Licensed Registered Nurse
